PRINTABLE DRIVER REFERENCE

After a roadside inspection.

Read the report. Notify your carrier. Follow the operating restrictions.

Driver: send the complete report

  1. Read the report before leaving. Check the driver and unit identifiers and any out-of-service designation. Ask the inspector to clarify instructions you do not understand.
  2. Notify your carrier promptly. Tell dispatch or safety whether the driver or equipment is restricted. Send readable copies of every page through your company’s approved channel.
  3. Follow the report handoff requirement. Under §396.9, deliver the report at the next terminal or facility to the carrier and, where applicable, the intermodal equipment provider. If arrival is not scheduled within 24 hours, transmit it immediately.
  4. Do not dispatch an out-of-service vehicle. Required repairs must be satisfactorily completed. Follow any separate driver restriction as well. The report’s certification deadline is not permission to keep operating.
  5. Keep the evidence together. Preserve the original report and provide the unit number, repair location, and relevant company contacts. Do not alter the report.

Office: assign the follow-up

Arrange corrections, document the evidence, and complete the required certification within 15 days. Return the form when requested by the issuing State agency and retain the copy for 12 months. Read all agency instructions.
