Example only: one location, five driver files, three vehicle records, and a records-retrieval walkthrough. A real report names the review date, record period, categories, sample, and limits agreed with the client.

Vehicle inspection evidence was not located

Observed: The sampled trailer folder did not contain inspection documentation. Vehicle-level documentation was outside this example's review.

Action: Maintenance verifies the unit's inspection status and applicable documentation before the next dispatch decision.

How a finding closes

For the vehicle-document example, the maintenance lead supplies the report for the correct trailer. The reviewer compares identifiers and dates, documents what was verified, and closes the record gap within the agreed scope. That document check does not certify mechanical condition.

For a missing review note, a verbal statement that the review occurred is information to investigate. The finding remains open until the relevant evidence is located or the required work is completed and documented on its actual date.

What the customer receives

An agreed-scope summary, observed facts, supporting references where applicable, recommended actions, and a discussion of priorities. Corrective implementation and closure verification are included only when the proposal specifies them.

What the review can establish

A missing item in a sampled folder does not prove it is absent everywhere. Reports distinguish observed facts from items that still need verification, and identify work outside the review. A private review does not determine the outcome of an official audit.
