Use a current roster
Identify the carrier and driver using the name or internal identifier used in the records. Do not enter a Social Security number on this index. Identify the applicable qualification requirements and exceptions before marking a record not applicable.
Present is not the same as verified
A present document still needs the correct person, date, and applicable information. Mark uncertain items for verification rather than treating a visible file as complete. Note the document location and assign missing items to a person with a target date.
Keep the scope of the index clear
The sheet covers common DQ file contents. Medical evidence differs for CDL and non-CDL drivers. Hiring investigations and drug-and-alcohol processes can require other records and procedures. Check retention by record type; the index does not establish that the entire hiring process is complete.
